Form 4 - P.O. Box as a MAILING address? How?

Form 4 - P.O. Box as a MAILING address? How?

How do you do this on a Form 4?

I have a trust and box 2a is for where the item will be stored (my home address.) BUT, I cannot receive mail at my house, so how do get them to mail everything to a PO Box?

Form 1's say "mailing address", then "if ... is a PO Box, street address must be given." What do you do on Form 4's??

Well, that's your 'Address', not a PO.Box.

edit- Sorry, I didn't read your post right.
Put in your "PO.Box 123" & "567 N. Main St" on the same or subsequent line.

My PO Box is 5 minutes away, but in a different city, county, and ZIP code. So it wouldn't be that easy. :\

Form 4 will be mailed to your dealer, not you. Put in your home address.

Yeah, I didn't think they would ever have to mail me anything......but you never know. FedEx and UPS will deliver, so if something comes back RTS via USPS multiple times, maybe they'll try another carrier.
