Questions on my Form 1s - Going Trust Route

Any of you actually done the Trust route and got approvals back, I have a few questions...

I don't mean to beat a dead horse, I just get nervous filling this crap out as I always goof something up and don't want to get it sent back in 4-5 months...

Questions are as follows....

4a Name and Location of Original Manufacture

Do I put LARUE TACTICAL, LEANDER TEXAS or MULLEN TRUST, ST. LOUIS MO (looking at the exmple online, it says saysd ""OR"" and shows it either way... Which one is better/Works?

4e. 12" Barrel 4f. ??? What Overall Length ? Will be using a Magpul CTR stock if that matters... (asked this one on the Larue forum too)
4e. 10.5"Barrel 4f. ??? What Overall Length ?

4h. Additional Description (via google, I saw someone doing trust route got rejected for leaving this blank, examiner said ""Must put Trust info as engraved") so do I put

ST. LOUIS. MO

Which also brings me to the next question... Neither of my lowers say a Caliber (Larue says Multi-Caliber, Noveske says, well it doesn't say anything regarding caliber) So under the ""Trust name, Location, should I add a 3rd line that says Caliber 5.56MM or 5.56MM or not add?

I think thats it for now...

I figured I'd try here 1st before the NFA forum, hoping some of you guys already did this...

I will give you some info based on my last approved F1 (individual)

4A. Original manufacturer of the lower, e.g. Larue, Noveske, CMMG, Whatever. My form 1 lists CMMG INC. FAYETTE, MO
4E. Educated estimate; i looked up a few known lengths based on similar designs, and used the difference of a 16.5" barreled upper on a lower with the same stock and came up with roughly the same difference (16.5" upper on same lower config should be ~5" shorter than a 11.5" upper.) OAL is a pain. Off the top of my head my 11.5" with CTR is 28" OAL with stock extended.
4H. Should have what you listed there. Mine lists 'NAME LASTNAME CITY MO' this doesn't change for trusts

List what is marked on the lower for caliber, or list 5.56. You can change the caliber/length/whatever anytime you want and you don't have to notify the ATF of changes as long as it is a 'temp' change (you have the ability to change it to it's approved form)

Also as for errors. I had a screw up awhile back and the examiner returned the form to me, I corrected it and had the stamp back inside 2 weeks.

LARUE is the Original Manufacturer and not you you are just turning it into something else but again you did not creat the original Lower

you are the maker of it - you Trust is what will be engraved onto your Lower - Trust Name - STL, MO

one of the Lowers that I am waiting on is for a 12.5 Inch SBR and that is listed as 30 inches so just take o.5 inches of and that will bring it to29.5 inches.

I have a 10.5 Barrel on one of my SBR's and the same Stock and it is 28 inches

I have done now a total of 8 Form 1's and always left that area blank and all have been approved

you can put into the C. box 5.56. or 6.8SPC or 458 SOCOM - these are left blank so you can use it for any caliber. There are several Companies that do not put anything onto the Lower about Caliber. You do not need to engrave this info just your TRUST NAME AND CITY AND STATE
